Subject: FYI: changing the name of ADA DM to SUD DM

Hey Nora,

Jenn and I had a discussion regarding the “ADA” in “ADA DM” and in an effort to stay current with best practices and use the most appropriate language we are transitioning ADA DM’s name to SUD DM.

Please let me know if you have any questions, thanks!

Subject: Disease Management Projects

Good Morning Team,

In an effort to stay current with best practices and use the most appropriate language we would like to begin the transition of utilizing SUD DM in place of ADA DM.  The plan would be to start by emailing the DM Projects (SUD DM and DM 3700) Coordinators regarding the transition and include the attachment above.  Immediately after that all of our documents, websites, and emails with information regarding the DM Projects will be updated.

Please let me know if you have any questions or concerns regarding the plan for this transition.

Thank you,

Jessica Bounds
