For what it's worth (I was on vacation last week and am catching up on e-mail!), we currently use EBSCO AtoZ with Linksource.  Until January 2009, we used ExLibris Group's a to z, along with SFX as the link resolver.

There has definite interest in our comparison of A-Z list products and so we have decided that the best way to gather information is to ask 10 questions in a survey form.
