Survey invitation: tracking perpetual access entitlements

If you are involved with managing electronic resources at an academic library, please consider participating in this survey concerning the tracking of perpetual access entitlements for electronic resources. The very brief survey will only take a few minutes to complete, and you will not be asked for any personally identifiable information. Take the survey here: https://siue.us2.qualtrics.com/SE/?SID=SV_eDHW0JOlwrlxXb7

The survey contains questions about purchasing journal, book, and multimedia content with perpetual access provisions, and tracking those resources. For a little background, I presented on this topic at the Charleston Conference recently, and the slides are available here: http://www.slideshare.net/chrisbulock/you-call-that-perpetual-issues-in-perpetual-access

While some excellent research has been done regarding perpetual access provisions in license agreements, particularly for journal content, I am interested to see whether libraries are tracking and taking full advantage of that access. I plan to publish the results of this research, but feel free to contact me prior to publication to discuss the results.
