Maureen,

Our commercial binder provides these to us as part of the cost of binding.
(e.g., as he returns a vol. of "Newsweek", he also leaves us 1-2
pre-printed multi-part forms for future vols. of the same title).

He also routinely gives us totally blank forms, just for initial binding
instructions on titles that may be new to us.

We find the multi-parts are still very useful.

Maureen,

Our multi-part bindery forms are provided by our commercial binder, The
Heckman Bindery, but are produced by Moore Corporation (www.moore.com).
Hope this helps.

We are looking for a vendor for multi-part binding forms. Do you still use
these? They are the forms that note the color and spine wording. We want
multipart forms so we can keep a copy for our records. Any other creative
(i.e. more technologically up to date ) solutions are also welcome.
