The CONSER Cataloging Manual, Module 14.1.4 -- Components of a linking
entry field states that 245 $a $n $p are transcribed in linking field $t
and then provides a single Common title/section title example:

  780 00  $t Fish management report. Northeast, Northwest, and Central
          Florida regions

In Cataloger's Desktop, the CCM also provides a link to the CONSER
Editing
Guide, Linking Entry Fields section which provides a little more detail:

$t  Title.
Contains the title from field 130 (Uniform title heading) or field 245,
subfields $a, $n, and $p.

Related record:
245 00  Journal of polymer science. $n Part C, $p Polymer symposia.

Linking entry:
[765-787]  $t Journal of polymer science.  Part C, Polymer symposia
