At Boise State University we've cataloged a number of ejournals and
such.  Our note is ambiguous, and intentionally so.  It say

"Boise State University access only"

For most titles that means on campus, ip authenticated access only.
For some there is off campus access to authenticated users.  And, a
few are open access to anyone.  By keeping it ambiguous we don't try
to explain every conceivable option, much less need to change the note
every time something changes on a journal. And, someone could be at
another campus or business that also has ip authenticated access to
the title, so it would work for them too.

For an example, visit http://catalyst.boisestate.edu/ and find the
journal title "Death Studies".  Also, note that the URL there does NOT
go directly to the journal, but through an interim page that gathers
statistics. I can provide more info on that if anyone wishes.

After much discussion among reference, systems, and cataloging, we came
up
with the 590 note:

"Access is restricted to NMSU users only. Please see
http://lib.nmsu.edu/resources/offcamp.html for more information and for
off-campus access options."

The link goes to our web page, which explains about some resources being
limited to persons affiliated with the University, and then explains
about
off-campus access options. We had tracked complaints from people who
could
not access our resources, and it turned out that most were from people
who
were having problems getting in from off-campus, and this solved both
problems with one brief note.
