Subject: Looking for a Good User Survey Model

Does anyone know of a good place to find a concise, easy to understand,
survey of traditional library services that has been published in the past
five years or so?

Ideally, I'd like to see a list of no more than 15 or 20 questions at the
most, asking users to rate collections, access, the building itself, staff,
etc.
