We have notes in our records that the publisher will provide free access
as long as we're willing to get in with a user name and password, but
that
if we want IP access, we have to pay extra.  (We opted not to do this
since we have full text from ProQuest.)

password?  I remember no end of strange access problems/denials of

from publishers when I was working on this sort of thing a few years
ago,
and in almost all cases, it was something at the publisher's end that
was
causing the problem.
