Good afternoon!

I am hoping that someone can help me.  I am trying to order a journal and
EBSCO cannot handle it for us.  I was told that I needed to order it from
the publisher.  One problem, I do not nor can I find the address  and phone
number.  If anyone has this title, could you please tell me how you ordered
it.  Or if anyone knows the address and/or phone number, I would be forever
in your debt.

        Publisher:  Professional Health Services.  Princeton, NJ 08543-5273.

This is the only info I have.  Thanks for any help!
