We also try our hardest to replace the missing pages with photocopies,
even color items, unlesss the subject librarian says not to replace.  When
the number of pages are to great to just tip in, we will try and replace
the whole item, bound or unbound, cost and subject librarian approval
permitting.  If either option is not available we will keep the mutilated
item with a note stating what is missing and sorry for the inconvience.

Hope this helps but it sounds exactly like what you already are doing.

One other thing, if the missing pages cannot or will not be replaced and
they are from a title we receive on microform, we include the call number
of the microform in the note.
