Mitch,

With the second copy of vol. 6, no. 1 (1995/1996), we received a letter from
the publisher explaining that it was a replacement copy which included 2
articles and an introduction which "were inadvertently left out of the
earlier version of the issue." The letter requested that we discard the
earlier version.
