Personally,I would prefer a replacement issue with a note to say that there
was an error and that the previous issue sent should be discarded.

We have experienced this situation only once or twice before and the
publisher has sent a completely new issue.

This makes it much easier to bind (and reduces binding costs too) rather
than trying to insert pages.
