I'm not either, and had I been the one designing the system it'd
probably have simply been "+1 difficulty on retries after an exceptional
failure (the referee should make up some explanation for this if
necessary)", or even just "exceptional failure means no retries until
the situation changes".

While there are strong warnings against overuse of task rolls, I don't
think the tasks were supposed to be always super-meaningful in the way a
lot of modern systems think they should (with their 'fail forwards' and
'partial success means succeed but at a price', etc.).
